For Resource Class (Boys and Girls)

·         Must submit a photocopy of his / her Birth Certificate

·         Photocopy of Developmental Pediatricians Assessment if available

·         Personal interview at the Principal’s Office

·         Pre-assessment and Screening at the Guidance Center

 

For Transition Class (Boys and Girls)

·         Must be at least 14 to 18 years old

·         Photocopy of Developmental Pediatricians Assessment if available

·         Personal interview at the Principal’s Office

·         Pre-assessment and Screening at the Guidance Center

 

For SPED ALS (Boys and Girls)

·         Must be at least 19 and above

·         Photocopy of Developmental Pediatricians Assessment if available

·         Personal interview at the Principal’s Office

·         Pre-assessment and Screening at the Guidance Center

 

For Kinder (Boys and Girls)

·         Must be at least four (5) years old by the opening of the school year;

·         Must submit a photocopy of his / her Birth Certificate

·         Photocopy of Developmental Pediatricians Assessment if available

·         Parent/ Relative Interview

 

For Elementary Level (Boys and Girls)

·         Must be at least five (6) years old at the opening of the school year.

·         Should pass the personal interview 

·         Must submit a photocopy of his / her Birth Certificate

·         Photocopy of Form 137 and Form 138

·         Photocopy of Developmental Pediatricians Assessment

·         Personal interview at the Principal’s Office

·         Pre-assessment and Screening at the Guidance Center
